{"doi":"10.3389/fnut.2023.1335927","title":"Editorial: Nutrition, sarcopenia, and sarcopenic obesity","abstract":"Word Count: 2838 immunosenescence, mitochondrial dysfunc�on, inflammatory disorders, chronic diseases, and changes in the intes�nal microbiome. Malnutri�on (undernutri�on, overnutri�on or nutrient deficiencies) contribu�ng to sarcopenia have been proposed for mul�ple classes of proteins, amino acids, lipids, carbohydrates, and micronutrients including vitamins and minerals.A par�cularly unique form of sarcopenia is the muscle loss and weakness that accompanies obesity, termed sarcopenic obesity (SO). This form of sarcopenia is also prevalent worldwide, par�cularly in older adults and in clinical se�ngs. Individuals with SO are at higher risk of adverse outcomes, such as func�onal impairment and hospitaliza�on.The overall goal of this special issue of Frontiers in Nutrition was to more clearly elucidate mechanisms contribu�ng to the development of sarcopenia, and sarcopenic obesity, to evaluate the impact of sarcopenia on other comorbid condi�ons, as well as to iden�fy op�mal therapeu�c strategies and dietary interven�ons for preven�on, reversal and/or management of sarcopenia.There were 46 manuscripts submited for this issue, of which 17 were published.The published manuscripts, as described below, focus mainly on the intersec�on of sarcopenia with other comorbidi�es including cancer, type 2 diabetes (T2D), Word Count: 2838 cirrhosis, kidney stones, severe abdominal trauma, and others. Several manuscripts examine impact of dietary inflammatory indices on the development of sarcopenia, while others explore the importance of nutri�onal supplementa�on with and without exercise training.In a cross-sec�onal study of 479 individuals, 264 (55.1%) men and 215 (44.9%) women, median age 71 years with T2D, Shiroma et. al (1) observed that 8.6% of pa�ents presented with sarcopenia. To evaluate undernutri�on status, they compared measurements of serum albumin, GNRI (Geriatric Nutri�on Risk Index) based on height, weight, and serum albumin, to CONUT (Controlling Nutri�on Status) based on serum albumin, total peripheral blood lymphocyte count, and total cholesterol. Their study found the diagnos�c power of GNRI to be superior to the measurement of albumin or CONUT for iden�fying sarcopenia, low skeletal muscle mass index, and low hand grip strength in pa�ents with undernutri�on and T2D. Authors discussed the possibility that sarcopenia in T2D may be caused by undernutri�on due to therapeu�c diets and/or an�-diabe�c agents.Researchers suggested the applica�on of the simple GNRI tool to iden�fy pa�ents in need of nutri�onal support for the improvement of sarcopenia. Word Count: 2838 Yang et. al (2) reported on a clinical-based observa�onal study of 217 pa�ents (68.6% male), 67.3 ± 11.1 years old, exhibi�ng diabe�c foot ulcer (DFU), followed over 4 years at Chongqing Medical University. All par�cipants underwent dualenergy X-ray absorp�ometry (DXA) to determine body composi�on. Thirty-three (17.3%) of 217 pa�ents died. The 5-year survival for the en�re group was 58.3%.Pa�ents with sarcopenia had a reduced 5-year overall survival (OS) of 45.9%, compared to 85.4% OS for pa�ents without sarcopenia. Age, sarcopenia, and serum crea�nine were independent risk factors for all-cause mortality in these pa�ents with DFUs. The authors suggest that ac�ve preven�on of sarcopenia may increase survival in pa�ents.Several groups reported on the impact of sarcopenia in pa�ents with cancer. Two studies focused on sarcopenia in pa�ents with lung cancer and another two in pa�ents with colorectal cancer (CRC). In a study of 126 newly diagnosed Stage III and IV lung cancer pa�ents, comprised of 97 (77%) men and 29 (23%) women, with a mean age 64.8 ± 8.7 years, Wang et al. (3) reported the prevalence of sarcopenia and frailty were 25.4% and 32.5%, respec�vely. They reported the presence of low body mass index (BMI) to be 40%, with low skeletal muscle mass measured by bioimpedance analysis (BIA) to be 38.1%.
