Peptide receptor radionuclide therapy (PRRT) is a process by which a somatostatin analog (octreotate) is combined with a chelator (DOTA) and a radionuclide (lutetium-177 [<sup>177</sup>Lu-dotatate]). This therapy targets receptors on neuroendocrine cells, causing internalization of the radionuclide by the tumor cell, which results in cellular damage and apoptosis. <b>Case Report:</b> We describe the clinical and therapeutic course of a 69-year-old male with a metastatic rectal NEC in whom progressive disease was noted after multiple therapies were attempted. After PRRT with <sup>177</sup>Lu-dotatate, the patient was asymptomatic and demonstrated a near-complete radiologic response. <b>Conclusion:</b> This case illustrates that treatment with PRRT may improve the outcome of patients with metastatic rectal NEC. Our case highlights the importance of further research into the use of PRRT in patients with a Ki-67 &lt;55% and uptake on somatostatin receptor imaging.","is_dataset_classified":null,"base_score":0.0,"endowment":0.0,"datacite_reuse_total":0,"file_count":0,"downloads":0,"views":0,"has_version_chain":false,"is_dataset":false,"is_oa":false,"pmid":"34566515","pmcid":null,"openalex_id":"https://openalex.org/W3201347213","authors":[],"funders":[],"total_grants":0,"fwci":0.0,"citation_percentile":0.1497498,"influential_citations":0,"citation_trend":[],"oa_status":"gold","license":"cc-by","oa_locations":[{"url":"http://www.ochsnerjournal.org/content/ochjnl/21/3/306.full.pdf","host_type":"journal"},{"url":"http://www.ochsnerjournal.org/content/ochjnl/21/3/306.full.pdf","host_type":"publisher"},{"url":"https://syndication.highwire.org/content/doi/10.31486/toj.20.0098","host_type":"publisher"},{"url":"https://doi.org/10.31486/toj.20.0098","host_type":"journal"},{"url":"https://pubmed.ncbi.nlm.nih.gov/34566515","host_type":"repository"},{"url":"http://europepmc.org/pmc/articles/PMC8442225","host_type":"repository"},{"url":"https://www.ncbi.nlm.nih.gov/pmc/articles/8442225","host_type":"repository"}],"fields_of_study":["Neuroendocrine Tumor Research Advances","Lung Cancer Research Studies","Neuroblastoma Research and Treatments"],"mesh_terms":[],"keywords":["Radionuclide therapy","Peptide receptor","Neuroendocrine tumors","Medicine","Somatostatin receptor","Asymptomatic","Somatostatin","Internalization","Oncology","Internal medicine","Nuclear medicine","Cancer research","Receptor","Carcinoma–neuroendocrine","gallium Ga 68 dotatate","lutetium-177","receptors–somatostatin"],"sdg_mappings":[{"sdg_number":0,"sdg_label":"Good health and well-being"}],"linked_datasets":[],"clinical_trials":[],"software_tools":[],"database_accessions":[],"source":"live","citation_network_status":"fetched"},"created_at":"2026-07-30T01:41:26.298442Z","pmid":null,"pmcid":null,"fwci":null,"citation_percentile":null,"influential_citations":0,"oa_status":null,"license":null,"views":0,"total_file_size_bytes":0,"version_count":0,"fair_f":null,"fair_a":null,"fair_i":null,"fair_r":null,"fair_zscore":null,"fair_rationale":null,"fair_model":null,"fair_agent_version":null,"fair_fulltext_source":null,"fair_has_llm":null,"fair_computed_at":null,"clinical_trials":[],"software_tools":[],"db_accessions":[],"linked_datasets":[],"topics":[]}
