The mechanisms underlying the lack of satiety in patients suffering PWS remain unknown and, hence, no etiological treatments have been approved. Different studies have proposed that deregulation of brain circuits controlling feeding contribute to the insatiable hunger and associated behavioral alterations linked to PWS (1). Although identification of effective targets for novel therapies remains as an unmet medical need, candidates have recently emerged, including ATP-sensitive potassium (KATP) channels (2) and the neuropeptide oxytocin (3). Diazoxide is a KATP channel activator, shown to improve hyperphagia and metabolic phenotype in obese animal models, including the PWS-model, Magel-2 null mouse (2, 4). Diazoxide can cross the blood-brain-barrier, reaching and inhibiting hypothalamic orexigenic neurons that co-express neuropeptide Y and agouti-related protein, thereby attenuating hyperphagia (2). In good agreement, a pilot phase 2 study with diazoxide choline controlled-release tablet (DCCR), formulated as oral, once-a-day extended-release pill, preliminarily documented a decrease in hyperphagia, aggressive behavior and body fat in patients with PWS (5).
