{"doi":"10.1111/bjh.16372","title":"Histiocytic sarcoma arising in patient with history of clonally‐related germ cell tumour and myelodysplastic syndrome","abstract":"One year after chemotherapy, a 26-year-old man with a history of metastatic non-seminomatous germ cell tumour with yolk sac component (NSGCT) (top left, ×20 objective) and mutations in TP53, BCOR and RRAS2, along with isochromosome 12p, i(12p), presented with myelodysplastic syndrome (MDS) (top right, CD61 highlighting micromegakaryocytes, ×40) with a complex karyotype including i(12p) and the same TP53 and BCOR mutations. The patient received an allogeneic stem cell transplant (ASCT) and achieved complete remission. A year after ASCT, imaging studies showed a right humerus lesion that revealed a histiocytic sarcoma (bottom left, ×20) with CD68+ (bottom right, ×20), CD33+, TP53+, lysozyme+, CD4+, CD11c−, CD14−, S100−, CD1a−, BRAF− and CD163−. Molecular studies identified 25% host component and the same TP53 and BCOR mutations, and in addition NRAS and RARA mutations. Sequencing studies of patient-matched tumour and normal samples were performed to identify somatic alterations. Follow-up imaging revealed other lesions that were biopsied and showed NSGCT.
