{"doi":"10.1093/sleep/zsad116","title":"Night-to-night reliability and agreement of physiological endotypes: two steps forward, one step back","abstract":"Despite many studies examining the relationship of physiological endotypes (loop gain, arousal threshold, upper airway collapsibility [Vpassive], and muscular compensation [Vcomp]) to sleep apnea pathogenesis [1–4], research has only recently begun to examine the foundational question of whether these characteristics are reproducible within a given individual [5–7]. If not, the clinical utility of these physiological endotypes is questionable. In the current issue of SLEEP, Tolbert et al (Sleep. 2023;46(8):zsad058. doi: 10.1093/sleep/zsad058) used the Phenotyping Using Polysomnography method [1–3] to derive endotypes during non-rapid eye movement (NREM) sleep from two polysomnography studies obtained on consecutive nights in 43 older (56–76 years), community-dwelling, and largely non-sleepy volunteers, and then examined the night-to-night reliability and agreement. In short, the authors find that while most endotypes are reliable from night-to-night (e.g. individuals have similar ordering relative to one another), there was substantial disagreement for all measures (e.g. large within-subject changes in values measured on consecutive nights).
