{"doi":"10.1093/oncolo/oyab039","title":"Survival According to Primary Tumor Location, Stage, and Treatment Patterns in Locoregional Gastroenteropancreatic High-grade Neuroendocrine Carcinomas","abstract":"BACKGROUND: Although the gastrointestinal tract (including the pancreas, gastroenteropancreatic (GEP) is the most common site for extrapulmonary neuroendocrine carcinoma (NEC), the current treatment patterns of locoregional GEP NEC and in particular, the role of surgical resection is unclear. METHODS: Data from the National Cancer Database between 2004 and 2016 were used for this study. RESULTS: Of 2314 GEP NEC cases (stages I-III), 52.5% were stage III. Colon was the most common site (30%); 30.9% of all cases were small cell morphology. Age, morphology, stage, and primary site were associated with significant differences in treatment patterns. Management of NEC mimicked that of adenocarcinomas arising at the respective sites: colon NEC most likely to be treated with surgery and chemotherapy; anal and esophageal NEC was primarily likely to receive chemotherapy and radiation, and rectal NEC mostly likely to receive trimodality therapy. However, 25%-40% of patients did not undergo surgical resection even at sites typically managed with curative resection, and there was a trend toward lesser resection over time. The prognostic impact of surgical resection was significant across all stages and correlated with variations in survival across primary sites. Even in patients undergoing chemoradiation, surgery was the only prognostic variable that significantly affected survival in stages I-II patients (HR 0.63) and showed a strong trend in stage III (HR 0.77) patients. CONCLUSIONS: Treatment patterns in GEP NEC vary considerably according to stage and primary tumor site. Surgery significantly improved survival in stages I-II patients and showed a strong trend in stage III patients regardless of primary tumor location and other perioperative therapies.","journal":"The Oncologist","year":2022,"id":240127,"datarank":1.3633229477375446,"base_score":3.8501476017100584,"endowment":3.8501476017100584,"self_citation_contribution":0.5775221402565088,"citation_network_contribution":0.7858008074810359,"self_endowment_contribution":0.5775221402565088,"citer_contribution":0.7858008074810359,"corpus_percentile":84.88435058404889,"corpus_rank":1955,"citation_count":46,"citer_count":42,"citers_with_citation_signal":31,"citers_with_endowment":31,"datacite_reuse_total":0,"is_dataset":true,"is_dataset_confidence":0.7787,"is_data_producer":false,"deposit_databanks":null,"is_oa":true,"file_count":0,"downloads":0,"has_version_chain":false,"published_date":"2022-01-01","fair_score":4.1667,"fair_percentile":4.891470498318557,"algorithm_id":"datarank_citation_only_1hop_v6","ranking_scope":"data_only","authors":[{"id":389839,"name":"Chan Shen","orcid":"0000-0001-5680-3134","position":1,"is_corresponding":false},{"id":868028,"name":"Anjali Devabhaktuni","orcid":null,"position":2,"is_corresponding":false},{"id":868029,"name":"Ruda Nighot","orcid":null,"position":3,"is_corresponding":false},{"id":867261,"name":"Halfdan Sørbye","orcid":"0000-0002-7132-6214","position":4,"is_corresponding":false},{"id":274183,"name":"Arvind Dasari","orcid":"0000-0001-5271-9744","position":0,"is_corresponding":true}],"reference_count":24,"raw_metadata":null,"created_at":"2026-07-19T00:22:46.554789Z","pmid":"35380711","pmcid":"PMC8982433","fwci":null,"citation_percentile":null,"influential_citations":0,"oa_status":null,"license":null,"views":0,"total_file_size_bytes":0,"version_count":0,"fair_f":11.1111,"fair_a":18.75,"fair_i":0.0,"fair_r":25.0,"fair_zscore":-1.1986,"fair_rationale":{"fair_score":4.17,"has_llm":true,"taxonomy_version":"fair_taxonomy_v5","dimensions":{"F":{"name":"Findable","score":11.11,"criteria":[{"key":"f_dataset_pid","label":"Persistent identifier for the data","kind":"llm","weight":2.0,"fraction":0.0,"verdict":"no","evidence":null,"grounded":false,"rationale":"No persistent identifier for the study's own dataset is provided.","anchors":["RDA-F1-01D — FAIR Data Maturity Model: 'Data is identified by a persistent identifier' (priorit","RDA-F1-02D — FAIR Data Maturity Model: 'Data is identified by a globally unique identifier'","FsF-F1-02D — F-UJI/FAIRsFAIR: 'Data is assigned a persistent identifier'"],"scored":true,"signal":null},{"key":"f_repository_named","label":"Named repository","kind":"llm","weight":2.0,"fraction":0.0,"verdict":"no","evidence":"The data underlying this article will be shared at reasonable request to the corresponding author.","grounded":true,"rationale":"The paper does not name a repository; it only names the corresponding author, which is not a host.","anchors":["RDA-F4-01M — FAIR Data Maturity Model: metadata is offered so it can be harvested and indexed (","NIH DMS Policy Element 4 (NOT-OD-21-014) — name the repository where data will be archived","NSTC Desirable Characteristics of Data Repositories (2022) — 'Long-Term Sustainability', 'Reten"],"scored":true,"signal":null},{"key":"f_data_availability_statement","label":"Data-availability statement","kind":"llm","weight":2.0,"fraction":0.5,"verdict":"partial","evidence":"The data underlying this article will be shared at reasonable request to the corresponding author.","grounded":true,"rationale":"The data‑availability statement points to a person for requests, which is Colavizza category 1 (data available on request).
