(Focus group participant commenting on a tobacco education ad for LGBTQ+ young adults) A one-size-fits-all approach to reducing commercial tobacco use is not expected to resonate across diverse populations within the Lesbian, Gay, Bisexual, Transgender, and Queer (LGBTQ+) community as it could exacerbate tobacco-related health disparities due to inequities in uptake of prevention and cessation interventions among LGBTQ+ subpopulations. This commentary highlights the authors’ discussions arising from a Society for Research in Nicotine and Tobacco webinar “Does my intervention need an intervention? Taking stock of culturally tailored approaches to preventing and reducing commercial tobacco use among sexual and gender minoritized populations.” We define the concept of cultural tailoring, provide an overview of the theory and rationale underlying cultural tailoring, highlight the unique experiences of individuals with intersecting identities, discuss the complexity of balancing identity markers in culturally tailored messaging, explore ethical implications, and propose a call to action for a multilevel approach for tailoring tobacco public education messages and interventions for the LGBTQ+ community. Scientists and practitioners are developing culturally tailored interventions to address persistent tobacco disparities among LGBTQ+ populations.1–3 Cultural tailoring refers to adapting a health message or intervention for a population by incorporating their identities, values, needs, and cultures. Kreuter and colleagues4 operationalized cultural tailoring into five strategies: (1) peripheral, (2) evidential, (3) linguistic, (4) constituent-involving, and (5) sociocultural. For instance, adopting these five strategies for LGBTQ+ audiences may involve (1) using LGBTQ+ symbols and images as peripheral cues, (2) presenting evidence of smoking-related health impacts on LGBTQ+ individuals and community, (3) using terms and phrases that resonate with the LGBTQ+ community, (4) portraying messengers who identify as LGBTQ+, and (5) describing smoking-related risks in the context of the broader social and cultural values of the LGBTQ+ community.5 Prior interventions have leveraged nonsmoking LGBTQ+ peer support networks,6–8 organizational and community leaders,6–8 and social branding8,9 to engage LGBTQ+ audiences and encourage tobacco prevention and cessation. Scoping reviews suggest that LGBTQ+-tailored tobacco cessation and prevention interventions are undertheorized.1,2 McQuoid and colleagues found that, of 15 published interventions, five did not have an explicit theoretical basis. Individual-level theories of behavior change predominated, including the Transtheoretical Model and the Theory of Reasoned Action.1 A minority of interventions referenced frameworks explicitly engaging with LGBTQ+ people’s social contexts, namely, Theory of Diffusion of Innovations and the Minority Stress Model.1 In terms of effectiveness, only four included a comparison group, making it difficult to assess outcomes across interventions.1 Available evidence for LGBTQ+-tailored tobacco intervention efficacy is mixed, with only one intervention significantly affecting abstinence.1 Frameworks employed thus far address important within-person factors related to smoking cessation but may not adequately address drivers of LGBTQ+ tobacco use that reside in LGBTQ+ people’s social, living, and working environments. LGBTQ+-tailored tobacco interventions may be advanced by leveraging not only the strengths of individual-level behavior change theories, but also frameworks that acknowledge LGBTQ+ tobacco use inequities as indivisible from place, context, and policy.
