{"doi":"10.1093/neuros/nyab308","title":"In Reply: A Novel Framework for Network-Targeted Neuropsychiatric Deep Brain Stimulation","abstract":"To the Editor: We thank the authors1 for their thoughtful comments and agree regarding the additional host of opportunities available for implementing surgical neuromodulation techniques such as deep brain stimulation (DBS) for refractory neurological and psychiatric disorders. As stated in our paper,2 and as emphasized by the commentary from Brown et al,1 DBS is being increasingly used for various neurological disorders. However, while there exist plentiful opportunities for the application of DBS therapy, we must acknowledge that results from ongoing trials with DBS therapy for these myriad disorders have shown mixed results. A critical limitation of using DBS for improvement or “neuroenhancement” is the incomplete understanding of the neural dynamics of the dysfunctional network(s) underlying the disease being treated. While there has been substantial progress in understanding neural correlates of movement disorders such as Parkinson's disease, a similar degree of robust understanding of the neurophysiology of psychiatric disorders largely remains to be accomplished. Of the many ways to accomplish this necessary network-level understanding, our article describes one platform that we feel would be of use to the neuromodulation community. As a natural extension of the epilepsy surgery approach, it is familiar to the field and can be adapted as needed for each specific new application.
