In spite of considerable research progress, both voiding problems and incontinence continue to remain underdiagnosed, undertreated, and at times managed in a suboptimal manner using medications known to be associated with considerable adverse effects as a result of their established anticholinergic properties (1,3). Many factors contribute to the challenges confronting clinicians in managing this common and frequently disabling condition. In addition to the social stigma associated with incontinence, and general under-reporting of symptoms by patients, many clinicians working in primary care settings have been stymied by lack of quality education, together with a number of “cognitive disconnects.” For example, although compounds with anticholinergic properties have been recommended for symptoms now considered to be part of overactive bladder since the late 19th century (4), expert panels consider these types of medications to be risky and potentially inappropriate for use in older patients (3). Other challenges include that although most clinicians have traditionally considered incontinence to be a bladder problem, growing evidence indicates that in older adults urinary incontinence and related voiding problems present as a geriatric syndrome with complex multifactorial etiologies involving predisposing and precipitating risk factors reflective of considerations pertaining to factors as disparate as mobility, fluid balance, cognition, and neural control (1,5). Also, although multicomponent interventions involving behavioral elements and exercise have been shown to be safe and effective (6), U.S. reimbursement issues have presented challenges in terms of clinical implementation.
