{"doi":"10.1093/gerona/glac163","title":"Delirium and Cortical Complexity","abstract":"To the Editor—With great interest, we read the recent study by Tanabe et al. (1), which demonstrated reduced electroencephalographic (EEG) complexity, assessed via the Lempel-Ziv algorithm (2), during postoperative delirium. The study results also revealed a negative correlation between global Lempel-Ziv complexity and delirium severity in broadband (0.1–50 Hz) EEG. Posterior-temporal complexity is also negatively correlated with monocyte chemoattractant protein-1, suggesting that inflammation may play a pathogenic role with delirium precipitation, particularly in posterior regions of the brain that contain important hubs for arousal and consciousness (3). Overall, the results suggest that delirium reflects a constrained repertoire of brain states, with the corresponding loss of cortical information transfer that supports waking consciousness. This work is important, as our incomplete pathophysiologic understanding of delirium tempers the development of neurobiologically informed therapies.
