Epidemiology and clinical profile of pediatric burn injuries: A prospective study from Georgia","abstract":"Abstract Background Pediatric burn injuries remain a major public health concern, particularly in low- and middle-income countries. Despite the burden, epidemiological data from Eastern Europe are scarce. This study aimed to characterize the clinical, demographic, and epidemiological features of pediatric burn injuries in Georgia. It was conducted under the iCREATE initiative, funded by the NIH – Fogarty International Trauma Training Program (No. 2D43TW007261-11). Methods A prospective cross-sectional study was conducted at Simon Khechinashvili University Clinic, Tbilisi, from July 1, 2022, to January 31, 2023. All hospitalized patients aged ≤18 years with burn injuries were included. Data collected encompassed demographics, burn etiology, severity, anatomical distribution, treatment modalities, and outcomes. Statistical analyses included chi-square, Fisher’s exact, and non-parametric tests for group comparisons, with ordinal logistic regression used to identify predictors of hospital length of stay (LOS). Results Among 114 patients, the mean age was 3.68 ± 4.04 years, with 76.3% under five. Scalds were the predominant mechanism (80.7%). The upper extremities were most commonly affected (79.1%), and burns frequently involved multiple regions. Cold water was the most common first aid (51.9%), though one-third of patients received no pre-hospital care. Total body surface area (TBSA) ranged from 1% to 50% (mean 10.49% ± 7.7%). Surgical intervention was required in 11.4% of cases, and 14.0% needed resuscitation. TBSA (p = 0.039) and the need for resuscitation (p = 0.010) emerged as significant predictors of LOS in the ordinal logistic regression model. Conclusion Pediatric burns in Georgia primarily affect very young children and are largely scald-related. Burn severity and resuscitation needs are key determinants of hospital stay duration, underscoring the importance of prevention, early intervention, and improved access to specialized care.
