{"doi":"10.1093/cid/ciac067","title":"Reply to Ito and Ogawa","abstract":"To the Editor—We would like to thank Ito and Ogawa for their considerate Letter to the Editor regarding our manuscript on the comparative effectiveness of itraconazole and voriconazole in histoplasmosis [1]. To date, this is the only study comparing clinical effectiveness of voriconazole to itraconazole, the current standard of care [2]. We believe this is an important comparison, because voriconazole is frequently used due to, or at least the perception of, fewer drug interactions and easier to obtain drug levels [3]. However, there is a biologically plausible mechanism for voriconazole to potentially have lower effectiveness. Fluconazole has a chemical structure closely related to voriconazole and is known to have a lower barrier to resistance in histoplasmosis and a higher failure rate in the first 6 weeks of therapy [4, 5].
