To the Editors—We thank Schim van der Loeff and colleagues [1] for their interest in our article, and we welcome the opportunity to respond. These authors posit that it may not be possible to properly assess the contribution of other variables (including previous syphilis) if one variable (treatment for the index syphilis episode before entry into the study) is very highly associated with the outcome of interest. They suggest that we consider the influence of previous syphilis on detection of Treponema pallidum nucleic acid in blood and cerebrospinal fluid (CSF) separately in individuals who were not treated for uncomplicated syphilis before study entry.
