Weiss and colleagues [1] report the results of a pilot study applying a new brain imaging technique to the study of human immunodeficiency virus (HIV)–associated brain disease. Modern antiretroviral therapy, which controls viral replication and has revolutionized the expectation for duration and quality of life while living with HIV, nevertheless seems unable to fully normalize the performance of the brain. Systematic studies of patients with HIV, even when their HIV infections seem to be optimally controlled, continue to demonstrate that the quantitative performance on neuropsychological test batteries is less than predicted for their education, age, and other identified factors. Understanding this shortcoming of our current HIV therapy and seeking to correct it remain major goals of neuro-AIDS research.
