{"doi":"10.1093/cid/ciaa1849","title":"Vancomycin Duration of Therapy Can Inform the Need for Area Under the Curve Monitoring","abstract":"To the Editor—We read the recent publication by Dilworth et al regarding the utility of vancomycin advanced therapeutic drug monitoring (TDM) with interest. The authors identified a number of challenges related to performing area under the curve (AUC) concentration monitoring for vancomycin. They suggest that few patients receive vancomycin beyond 3 days and posit that short therapy might be associated with less toxicity, thus making advanced TDM a potentially futile endeavor [1]. Despite heterogeneous treatment durations, vancomycin-induced kidney injury (VIKI) is common and the impact of treatment duration is still under investigation. Notably, animal models have demonstrated that early injury occurs at even low, allometrically scaled doses [2, 3]; however, the recognition of VIKI with serum creatinine tends to occur after 48 hours of therapy [4–7].
