{"doi":"10.1093/bjs/znac393","title":"Transfusion of salvaged red blood cells during pancreatic ductal adenocarcinoma operations","abstract":"Allogeneic donor blood (aRBC) transfusion rates range from 26 to 75 per cent during pancreatic surgery1. aRBC transfusion is independently associated with nosocomial infections, acute lung injury, cardiac dysfunction, surgical morbidity, mortality, and recurrence-free and overall survival. Surgeons have nevertheless remained resistant to intraoperative blood salvage because of concern about metastases2. This belief is undermined by current understanding of metastases, and the early and continuous finding of circulating tumour cells (CTCs) in the blood of patients with cancer3,4. This experimental study aimed to compare the frequency of CTCs from pancreatic ductal adenocarcinoma (PDAC) in systemic preoperative, systemic intraoperative, and filtered salvaged red blood cells (sRBCs). The hypothesis was that there would be no difference between groups. Given that entry into the biliary and alimentary tracts can contaminate salvaged blood, microbial cultures were compared between the bile duct and sRBCs as a secondary objective.
