We characterized relationships between genetic polymorphisms and plasma efavirenz concentrations among 45 Haitians who initiated antiretroviral therapy in Port-au-Prince. METHODS: An observational study characterized relationships between clinical factors, pharmacokinetics, and treatment response among antiretroviral-naive patients initiating once-daily treatment with efavirenz plus twice-daily treatment with zidovudine and lamivudine. Plasma drug concentrations were determined at weeks 2 and 4. Drug doses were directly observed by field workers or designated family members. We retrospectively characterized relationships between efavirenz concentrations and 50 single-nucleotide polymorphisms in CYP2B6 and several polymorphisms in CYP2A6, CYP3A4, CYP3A5, and ABCB1. RESULTS: Plasma specimens for efavirenz analysis were obtained from study participants a mean (+/- standard deviation) of 13.9 +/- 1.6 h after they received the dose. As expected, CYP2B6 516G-->T was associated with increased plasma efavirenz concentrations (Spearman rho = 0.71; P < .001), as were 10 polymorphisms in linkage disequilibrium with 516G-->T. Distinct CYP2B6 polymorphisms were associated with decreased plasma efavirenz concentrations (greatest absolute rho = 0.48; P = .001). Associations were replicated by results from a recent pharmacokinetic study involving 34 healthy, human immunodeficiency virus-negative African Americans. CONCLUSIONS: Relatively frequent CYP2B6 polymorphisms may predict decreased plasma efavirenz exposure in patients of African descent. If replicated in other cohorts, the implications of these novel associations for treatment response warrant further study.","is_dataset_classified":null,"base_score":3.6888794541139363,"endowment":3.6888794541139363,"datacite_reuse_total":0,"file_count":0,"downloads":0,"views":0,"has_version_chain":false,"is_dataset":false,"is_oa":false,"pmid":"19659438","pmcid":"PMC2754599","openalex_id":"https://openalex.org/W2132698391","authors":[],"funders":[{"funder_name":"NIAID NIH HHS","grant_id":"AI 071205","title":null},{"funder_name":"NIAID NIH HHS","grant_id":"AI 077339","title":null},{"funder_name":"NIAID NIH HHS","grant_id":"AI 007046","title":null},{"funder_name":"NIAID NIH HHS","grant_id":"P30 AI050410","title":null},{"funder_name":"FIC NIH HHS","grant_id":"D43 TW000018","title":null},{"funder_name":"NIAID NIH HHS","grant_id":"P30 AI054999","title":null},{"funder_name":"NIAID NIH HHS","grant_id":"AI 064021","title":null},{"funder_name":"NIAID NIH HHS","grant_id":"AI 54999","title":null},{"funder_name":"NIAID NIH HHS","grant_id":"K23 AI077339","title":null},{"funder_name":"NIMH NIH HHS","grant_id":"R01 MH071205","title":null},{"funder_name":"NCRR NIH HHS","grant_id":"UL1 RR024975","title":null},{"funder_name":"NIAID NIH HHS","grant_id":"R21 AI064021","title":null},{"funder_name":"FIC NIH HHS","grant_id":"U2R TW006901","title":null},{"funder_name":"NIAID NIH HHS","grant_id":"T32 AI007046","title":null},{"funder_name":"FIC NIH HHS","grant_id":"TW 00018","title":null},{"funder_name":"FIC NIH HHS","grant_id":"TW 006901","title":null},{"funder_name":"NIAID NIH HHS","grant_id":"AI 50410","title":null},{"funder_name":"NCRR NIH HHS","grant_id":"RR 024975","title":null}],"total_grants":18,"fwci":3.035,"citation_percentile":0.90906157,"influential_citations":0,"citation_trend":[{"year":2012,"count":8},{"year":2013,"count":5},{"year":2014,"count":1},{"year":2015,"count":3},{"year":2016,"count":3},{"year":2017,"count":4},{"year":2018,"count":2},{"year":2022,"count":2}],"oa_status":"bronze","license":null,"oa_locations":[{"url":"https://academic.oup.com/jid/article-pdf/200/6/955/18059270/200-6-955.pdf","host_type":"journal"},{"url":"https://academic.oup.com/jid/article-pdf/200/6/955/18059270/200-6-955.pdf","host_type":"publisher"},{"url":"http://academic.oup.com/jid/article-pdf/200/6/955/18059270/200-6-955.pdf","host_type":"publisher"},{"url":"https://doi.org/10.1086/605126","host_type":"journal"},{"url":"https://pubmed.ncbi.nlm.nih.gov/19659438","host_type":"repository"},{"url":"https://www.ncbi.nlm.nih.gov/pmc/articles/2754599","host_type":"repository"}],"fields_of_study":["HIV/AIDS drug development and treatment","HIV/AIDS Research and Interventions","Pneumocystis jirovecii pneumonia detection and treatment","Adult","Alkynes","Anti-HIV Agents","Aryl Hydrocarbon Hydroxylases","Base Sequence","Benzoxazines","Cyclopropanes","Cytochrome P-450 CYP2B6","Female","HIV Infections","Haiti","Haplotypes","Humans","Linkage Disequilibrium","Male","Oxidoreductases, N-Demethylating","Polymorphism, Genetic"],"mesh_terms":["Adult","Alkynes","Aryl Hydrocarbon Hydroxylases","Base Sequence","Cyclopropanes","Female","Haiti","Haplotypes","Humans","Male","Oxidoreductases, N-Demethylating","Polymorphism, Genetic","HIV Infections","Linkage Disequilibrium","Anti-HIV Agents","Benzoxazines","Cytochrome P-450 CYP2B6"],"keywords":["Efavirenz","CYP2B6","Antiretroviral therapy","Human immunodeficiency virus (HIV)","Plasma concentration","Virology","Medicine","Internal medicine","Pharmacology","Viral load","CYP3A4","Metabolism"],"sdg_mappings":[{"sdg_number":0,"sdg_label":"Good health and well-being"}],"linked_datasets":[],"clinical_trials":[],"software_tools":[],"database_accessions":[{"name":"refsnp"}],"source":"live","citation_network_status":"fetched"},"created_at":"2026-08-18T16:21:08.130300Z","pmid":null,"pmcid":null,"fwci":null,"citation_percentile":null,"influential_citations":0,"oa_status":null,"license":null,"views":0,"total_file_size_bytes":0,"version_count":0,"fair_f":null,"fair_a":null,"fair_i":null,"fair_r":null,"fair_zscore":null,"fair_rationale":null,"fair_model":null,"fair_agent_version":null,"fair_fulltext_source":null,"fair_has_llm":null,"fair_computed_at":null,"clinical_trials":[],"software_tools":[],"db_accessions":[],"linked_datasets":[],"topics":[]}
