For successful ABI surgery, knowledge of the microanatomical variations and specific intraoperative findings are essential. Material and Methods: Between 1992 and 2004, 92 patients mostly suffering from NF 2 in 14 European and Asian countries were operated by the senior author. The translabyrinthine approach was used in 51 and the lateral suboccipital approach in 41 cases. The intraoperative positioning of the electrode carrier was guided by anatomical landmarks and monitoring of the auditory brainstem evoked potentials. Results: The mean age was 33 years, with tumor size 1.0 up to 6.0 cm in 78 NF 2 patients. Nine patients were free of tumor after previous AN surgery, 3 had hearing loss due to ossified cochlea, 1 suffered from axonal neuropathy, and 1 from a recurrent Lindau tumor. Intraoperative positioning of the electrode carrier was possible in 90 of 92 cases. Insertion of the implant was considered easy in 59 cases, difficult after extensive dissection in 31, and impossible in 2 cases. In 86 patients tested there were hearing impressions in 2 up to 21 electrodes (average 9.4). Most patients show tonotopic distribution of the stimulation responses. Conclusions: Constant anatomical landmarks are very helpful in finding the cochlear nucleus. The best results are obtained in patients with a stable course of their NF 2 disease; less favorable results are had in patients with large tumor recurrences and distortion or scar formation of the brainstem. Complications were related only to tumor surgery and not to dissection and insertion of ABI. The tonotopy of the CN can be used.
