METHODS: An electronic health record based algorithm is used to identify patients with T2D and either established atherosclerotic CV disease (ASCVD), high risk for ASCVD, chronic kidney disease, and/or heart failure within our health system. Patients are invited to participate and randomly assigned to either simultaneous education and medication management, or a period of education prior to medication management. Patient navigators (trained, non-licensed staff) are the primary points of contact while a pharmacist or nurse practitioner reviews and authorizes each medication initiation and titration under an institution-approved collaborative drug therapy management protocol with supervision from a cardiologist and/or endocrinologist. Patient engagement is managed through software to support communication, automation, workflow, and standardization. CONCLUSION: We are testing a remote, navigator-driven, pharmacist-led, and physician-overseen management strategy to optimize GDMT for T2D as a population-level strategy to close the gap between guidelines and clinical practice for patients with T2D at elevated CV and/or kidney risk.","journal":"Primary care diabetes","year":2024,"id":444115,"datarank":0.0,"base_score":0.0,"endowment":0.0,"self_citation_contribution":0.0,"citation_network_contribution":0.0,"self_endowment_contribution":0.0,"citer_contribution":0.0,"corpus_percentile":null,"corpus_rank":null,"citation_count":7,"citer_count":0,"citers_with_citation_signal":0,"citers_with_endowment":0,"datacite_reuse_total":0,"is_dataset":false,"is_dataset_confidence":0.9634,"is_data_producer":false,"deposit_databanks":null,"is_oa":true,"file_count":0,"downloads":0,"has_version_chain":false,"published_date":"2024-01-01","fair_score":null,"fair_percentile":null,"algorithm_id":"datarank_citation_only_1hop_v6","ranking_scope":"data_only","authors":[{"id":1259259,"name":"Lee‐Shing Chang","orcid":"0000-0002-8844-1219","position":1,"is_corresponding":false},{"id":859229,"name":"Caitlin Colling","orcid":"0000-0001-9062-8667","position":2,"is_corresponding":false},{"id":1259664,"name":"Gretchen Stern","orcid":null,"position":3,"is_corresponding":false},{"id":1259260,"name":"Daniel Gabovitch","orcid":"0009-0007-5854-4392","position":4,"is_corresponding":false},{"id":1259665,"name":"Guinevere Feldman","orcid":null,"position":5,"is_corresponding":false},{"id":1259261,"name":"Asma Adan","orcid":"0000-0001-8483-3322","position":6,"is_corresponding":false},{"id":1259262,"name":"Fanta Waterman","orcid":"0000-0002-5540-5641","position":7,"is_corresponding":false},{"id":1259263,"name":"Emily Durden","orcid":"0000-0003-3793-5240","position":8,"is_corresponding":false},{"id":1259264,"name":"Carol Hamersky","orcid":"0000-0002-7001-9562","position":9,"is_corresponding":false},{"id":1259666,"name":"Joshua Noone","orcid":null,"position":10,"is_corresponding":false},{"id":596067,"name":"Samuel Aronson","orcid":"0000-0002-2490-6822","position":11,"is_corresponding":false},{"id":1259667,"name":"Paul Liberatore","orcid":null,"position":12,"is_corresponding":false},{"id":501596,"name":"Thomas A.
