Swanton","orcid":null,"position":0,"is_corresponding":false}],"reference_count":0,"raw_metadata":{"has_enrichment":true,"resolved":true,"title":"A Critique of the American Urological Association Penile Prosthesis Antibiotic Prophylaxis Guidelines","abstract":"Guideline statements are necessary to provide a standardized, evidence-based approach to decrease the risk of perioperative infections while maintaining the principles of antibiotic stewardship. Both the American Urological Association (AUA) and the European Association of Urology (EAU) have published recommendations for antibiotic prophylaxis for urologic surgeries. However, the specific recommendations addressing prophylaxis for inflatable penile prosthesis (IPP) placement are inconsistent and in need of revision given our contemporary understanding of IPP infections. The purpose of this critique is to examine the current best practice recommendations for antibiotic prophylaxis prior to IPP placement and their adequacy in the context of current research. In its most recent iteration updated in 2019, the best practice statement published by the AUA recommends an aminoglycoside plus either a first or second generation cephalosporin or vancomycin in all prosthetic cases, including IPP placement. In patients with renal insufficiency, the best practice statement allows for substitution of aztreonam for the aminoglycoside. Alternate regimens, including potentially an aminopenicillin with a beta-lactamase inhibitor, are also recommended.
