Emerging evidence supports the beneficial effects of regular physical exercise (PE) on mental well-being. Recent advancements in wearable technology and physiological monitoring have provided novel opportunities to examine the physiological mechanisms underlying this relationship. This systematic review evaluates the literature regarding wearable physiological monitoring’s role in elucidating exercise’s effects on mental health. PE consistently has demonstrated positive impacts on mental health, particularly in improving mood, reducing anxiety and mitigating stress. Physiological markers frequently are employed to assess PE’s acute effects on psychological well-being. To conduct this review, relevant literature published between 2005 and 2024 was studied. Recommendations based on the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) model were adopted to identify, screen, validate and include relevant literature. The findings indicate that wearable physiological monitoring presents a promising approach to understanding the complex relationship between exercise and mental health. However, challenges remain in standardising measurement methods and determining optimal exercise parameters for mental health. More reliable and effective biomarkers should be identified to assess exercise’s impact on mental health in future research.
