The predictive ability of the model was analyzed with the area under the Receiver Operating Characteristic curve (AUC-ROC). The prognostic performance for true bacteremia was calculated with the cut-off values chosen for getting the sensitivity, specificity, positive predictive value and negative predictive value.
Results
A total of 3.843 blood samples wered cultured. True cases of bacteremia were confirmed in 839 (21.83%). The remaining 3.004 cultures (78.17%) were negative. Among the negative, 172 (4.47%) were judged to be contaminated. Low risk for bacteremia was indicated by a score of 0-2 points, intermediate risk by 3-5 points, and high risk by 6-8 points. Bacteremia in these 3 risk groups was predicted for 1.5%, 16.8%, and 81.6%, respectively. The model's area under the receiver operating characteristic curve was 0.930 (95% CI, 0.916-0.948). The prognostic performance with a model's cut-off value of ≥5 points achieved 94.76% (95% CI: 92.97-96.12) sensitivity, 81.56% (95% CI: 80.11-82.92) specificity, and negative predictive value of 98.24% (95% CI: 97.62-98.70).
Conclusion
The 5MPB-Toledo score is useful for predicting bacteremia in patients attended in hospital emergency departments for infection.","is_dataset_classified":null,"base_score":1.791759469228055,"endowment":1.791759469228055,"datacite_reuse_total":0,"file_count":0,"downloads":0,"views":0,"has_version_chain":false,"is_dataset":false,"is_oa":false,"pmid":"34992000","pmcid":null,"openalex_id":"https://openalex.org/W4220997548","authors":[],"funders":[],"total_grants":0,"fwci":0.6207,"citation_percentile":0.63368596,"influential_citations":0,"citation_trend":[{"year":2022,"count":2},{"year":2023,"count":2},{"year":2024,"count":1}],"oa_status":"closed","license":"https://www.elsevier.com/legal/tdmrep-license","oa_locations":[{"url":"https://api.elsevier.com/content/article/PII:S2529993X2100263X?httpAccept=text/xml","host_type":"publisher"},{"url":"https://api.elsevier.com/content/article/PII:S2529993X2100263X?httpAccept=text/plain","host_type":"publisher"},{"url":"https://doi.org/10.1016/j.eimce.2021.12.006","host_type":"journal"},{"url":"https://pubmed.ncbi.nlm.nih.gov/34992000","host_type":"repository"}],"fields_of_study":["Bacterial Identification and Susceptibility Testing","Sepsis Diagnosis and Treatment","Neonatal and Maternal Infections","Adolescent","Adult","Bacteremia","Blood Culture","Emergency Service, Hospital","Humans","Prospective Studies","ROC Curve"],"mesh_terms":["Blood Culture","Adolescent","Adult","Emergency Service, Hospital","Humans","Prospective Studies","ROC Curve","Bacteremia"],"keywords":["Bacteremia","Receiver operating characteristic","Medicine","Predictive value","Area under the curve","Internal medicine","Predictive value of tests","Prospective cohort study","Observational study","Positive predicative value","Antibiotics","Risk Score","Emergency Department","Predictors","Blood Cultures","Bacteraemia","Procalcitonin","Clinical Prediction Rule","Bacteriemia","Servicio De Urgencias","Procalcitonina","Factores Predictores","Hemocultivos","Escala Pronóstica","Modelo Predictivo"],"sdg_mappings":[{"sdg_number":0,"sdg_label":"Good health and well-being"}],"linked_datasets":[],"clinical_trials":[],"software_tools":[],"database_accessions":[],"source":"live","citation_network_status":"fetched"},"created_at":"2026-08-06T20:57:39.676188Z","pmid":null,"pmcid":null,"fwci":null,"citation_percentile":null,"influential_citations":0,"oa_status":null,"license":null,"views":0,"total_file_size_bytes":0,"version_count":0,"fair_f":null,"fair_a":null,"fair_i":null,"fair_r":null,"fair_zscore":null,"fair_rationale":null,"fair_model":null,"fair_agent_version":null,"fair_fulltext_source":null,"fair_has_llm":null,"fair_computed_at":null,"clinical_trials":[],"software_tools":[],"db_accessions":[],"linked_datasets":[],"topics":[]}
