Chemoimmunotherapy with bendamustine and rituximab has shown activity in a variety of lymphomas, and proteasome inhibitors have demonstrated pre-clinical synergy and early clinical activity in this population. The objectives were to determine the maximum tolerated dose of carfilzomib and the preliminary efficacy of this combination.<h4>Patients and methods</h4>The protocol followed a 3+3 design of carfilzomib dose escalation combined with standard doses of bendamustine and rituximab. Patients were treated for up to 6 cycles with an interim positron emission tomography/computed tomography after cycle 3.<h4>Results</h4>Ten patients were treated on the dose-escalation phase. The study was terminated at a carfilzomib dose of 56 mg/m<sup>2</sup>, and the maximum tolerated dose was not reached. The most common grade 3/4 adverse event was thrombocytopenia. There was 1 dose-limiting toxicity observed, grade 3 febrile neutropenia, and there were no treatment-related deaths. The overall response rate was 40% (complete response rate, 30%), with a median duration of response of 12 months and a median progression-free survival of 2.1 months.<h4>Conclusion</h4>Carfilzomib in combination with bendamustine and rituximab is a safe and well-tolerated treatment for patients with relapsed/refractory non-Hodgkin lymphoma. Preliminary data indicate that this combination may have efficacy with an acceptable side effect profile in this heavily pre-treated patient population with limited treatment options.","is_dataset_classified":null,"base_score":2.1972245773362196,"endowment":2.1972245773362196,"datacite_reuse_total":0,"file_count":0,"downloads":0,"views":0,"has_version_chain":false,"is_dataset":false,"is_oa":false,"pmid":"33478921","pmcid":null,"openalex_id":"https://openalex.org/W3115222117","authors":[],"funders":[{"funder_name":"National Comprehensive Cancer Network","grant_id":"","title":null}],"total_grants":1,"fwci":0.7973,"citation_percentile":0.72205692,"influential_citations":0,"citation_trend":[{"year":2021,"count":2},{"year":2022,"count":1},{"year":2023,"count":3},{"year":2024,"count":2}],"oa_status":"closed","license":"https://doi.org/10.15223/policy-004","oa_locations":[{"url":"https://api.elsevier.com/content/article/PII:S2152265020310417?httpAccept=text/xml","host_type":"publisher"},{"url":"https://api.elsevier.com/content/article/PII:S2152265020310417?httpAccept=text/plain","host_type":"publisher"},{"url":"https://doi.org/10.1016/j.clml.2020.12.020","host_type":"journal"},{"url":"https://pubmed.ncbi.nlm.nih.gov/33478921","host_type":"repository"}],"fields_of_study":["Lymphoma Diagnosis and Treatment","Multiple Myeloma Research and Treatments","Chronic Lymphocytic Leukemia Research"],"mesh_terms":["Rituximab","Bendamustine Hydrochloride","Adult","Aged","Antineoplastic Combined Chemotherapy Protocols","Diagnosis, Differential","Female","Humans","Lymphoma, Non-Hodgkin","Male","Middle Aged","Oligopeptides","Prognosis","Recurrence","Treatment Outcome","Drug Resistance, Neoplasm","Retreatment","Disease Management"],"keywords":["Medicine","Bendamustine","Carfilzomib","Rituximab","Oncology","Refractory (planetary science)","Hodgkin lymphoma","Internal medicine","Lymphoma","Lenalidomide","Multiple myeloma","Proteasome inhibitor","Mantle Cell Lymphoma","Relapsed Lymphoma","Refractory Lymphoma"],"sdg_mappings":[{"sdg_number":0,"sdg_label":"Good health and well-being"}],"linked_datasets":[],"clinical_trials":[],"software_tools":[],"database_accessions":[{"name":"nct"}],"source":"live","citation_network_status":"fetched"},"created_at":"2026-08-09T16:08:25.453684Z","pmid":null,"pmcid":null,"fwci":null,"citation_percentile":null,"influential_citations":0,"oa_status":null,"license":null,"views":0,"total_file_size_bytes":0,"version_count":0,"fair_f":null,"fair_a":null,"fair_i":null,"fair_r":null,"fair_zscore":null,"fair_rationale":null,"fair_model":null,"fair_agent_version":null,"fair_fulltext_source":null,"fair_has_llm":null,"fair_computed_at":null,"clinical_trials":[],"software_tools":[],"db_accessions":[],"linked_datasets":[],"topics":[]}
