{"doi":"10.1016/j.brs.2023.07.040","title":"Across-Scale Depression Severity Ratings in Transcranial Magnetic Stimulation Treatment","abstract":"Background: The Hamilton Depression Rating Scale (HAMD-17), Beck Depression Inventory (BDI), and Patient Health Questionnaire (PHQ9) are commonly-used scales to classify Transcranial Magnetic Stimulation (TMS) response and remission. In the absence of a standard scale, it is critical to know whether classifications vary across scales. Methods: This was a retrospective chart review of 100 patients with treatment-resistant Major Depression who received 30 or more Deep TMS treatments (standard depression protocol) augmented with intermittent Theta Burst Stimulation. Four depression classifications (minimal, mild, moderate, severe) were created based on published standards. HAMD-17, BDI, and PHQ9 classifications were compared at baseline and at endpoint and analyzed with chi-squared tests. Results: Depression classifications varied significantly among the HAMD-17, BDI, and PHQ9 scales at baseline and endpoint (p < 0.001). More patients were in less severe classifications on the HAMD-17 than on the BDI and PHQ9. Endpoint minimal (remission) classification rates were 61% on the HAMD-17, compared to 40% and 28% on the BDI and PHQ9, respectively. Endpoint severe classification rates were: 3% HAMD-17, 22% BDI, and 11% PHQ9. The BDI and PHQ9 differed significantly at endpoint (p < 0.05). Conclusions: Classification differences across the three scales were substantial and statistically significant at both timepoints. The HAMD-17 endpoint results were comparable to Brainsway’s HAMD-21 published rates, whereas the BDI and PHQ9 scores suggested poorer treatment responses. More research is needed to evaluate possible clinician (HAMD) and patient (BDI, PHQ9) rating biases and whether one of these (or another scale) can be considered a standard.
