{"doi":"10.1016/j.brs.2020.06.042","title":"Network-targeted Prefrontal Transcranial Magnetic Stimulation (TMS) in Neurodegenerative and Neuropsychiatric Disorders","abstract":"Background: There are many questions left to be answered about how best to perform TMS so that it is as effective as possible. Parameters that may be key for optimizing TMS to treat neurodegenerative and neuropsychiatric disorders include: where to place the coil in order to modulate the affected large-scale network(s), and whether intrinsic functional connectivity prior to TMS is sufficient to achieve a good outcome. We are currenting testing the efficacy of repetitive TMS to improve cognitive function in adults with mild cognitive impairment (MCI). Here, we examined pre-treatment connectivity of the left dorsolateral prefrontal cortex (L-DLPFC) with other brain regions. Design/Methods: Seed to whole brain connectivity was examined with respect to a L-DLPFC site previously identified for efficacious treatment of depression (Fox et al. 2012). Using fMRI data of 20 MCI participants, connectivity was measured between the L-DLPFC (centered on MNI -38, 44, 26) and all other voxels. Results: This L-DLPFC site showed functional connectivity with regions of the cingulo-opercular “salience” network—including anterior cingulate, paracingulate, central/frontal opercular, insular cortices, frontal pole, precentral, and posterior/anterior supramarginal gyri. There was anticorrelated connectivity between L-DLPFC and subgenual cingulate, as expected. Conclusion: With respect to rTMS as a potential treatment for MCI, this prefrontal site is part of a brain network that subserves executive functions. DLPFC stimulation has improved cognition in MCI/AD, as reported in early trials. In future work, we will examine pre- vs. post-treatment changes in connectivity to elucidate therapeutic mechanisms of rTMS for MCI and neurodegenerative disease.
