The presence of an aneurysmatic right atrium in the fetus is a life-threatening condition. High-risk factors like severe tricuspid regurgitation, great dilation of the right atrium and fetal hydrops are associated with a 69%-85% perinatal mortality. A 38-year old woman with a 20-week gestation was referred to us for a fetal unspecified heart defect with severe tricuspid regurgitation. The echocardiography showed a variegated and bizarre, poorly defined tricuspid valve with a wide-range motion of the valves, a massive tricuspid insufficiency, a dilated right ventricle with preserved contractility and an aneurysmatic right atrium. The right LHR ratio was normal. The pregnancy was uneventful and the fetus remained stable. The patient underwent Caesarean section in week 39. The 3 kg newborn needed neonatal resuscitation. The postnatal echocardiogram showed a 25 mm tricuspid ring with bizarre, rough valvar edges with to-and-fro motion inside the ventricle and back to the atrium, a large deficit of valvar coaptation, massive tricuspid insufficiency, aneurysmatic right atrium, right ventricle dilation with preserved contractile function and apparently small left ventricle. The chest X-ray showed a wall-to-wall cardiac image. Despite intense treatment, the infant's condition quickly worsened and she developed a cardiogenic shock, preventing any invasive treatment. She died at 23 hours of life. The necropsy showed no tricuspid valve, which was substituted by several disorganised, spongy bodies of tissue swellings hanging from the annulus. Published high-risk anatomic data include: tricuspid annular dilation, involvement of the 3 leaflets with nodular thickening and curling of the edges and right ventricle dilation with preserved contractile function, all present in our patient. Conclusion: Our patient had high risk factors despite the absence of hydrops.
