To the Editor: Cousiens et al. present an important model of changes in the hepatitis C epidemic among persons who inject drugs based upon scenarios of improved diagnosis, linkage to care, and access to treatment in the era of effective direct‐acting antiviral agents.1 Their model predicts reductions in transmission of hepatitis C at 10 years with access to treatment earlier in the course of chronic hepatitis C infection. Many individuals who inject drugs have the disease of opioid use disorder. This is a disease for which we have highly effective therapy with methadone, buprenorphine, or injectable naltrexone. We are concerned that their model does not include treatment for opioid use disorder, which is known to reduce hepatitis C transmission.2 This issue is even more relevant for the United States, where we are experiencing an epidemic of opioid use disorder and have far less availability of buprenorphine than in France. In France any doctor can prescribe buprenorphine.3 In the United States physicians must obtain a specific waiver to prescribe buprenorphine; in 2011 there were on average just seven prescribers per county.4 Infectious disease physicians, hepatologists, and other physicians providing care for persons who inject drugs should also lower the risk of hepatitis C infection or reinfection after effective treatment by addressing the underlying opioid use disorder, either through directly prescribing medication‐assisted therapy or directly linking to another physician who prescribes medication‐assisted therapy. Cousiens et al. note that treatment of hepatitis C infection is not the only way to reduce transmission. In modeling the care cascade for hepatitis C infection among persons who inject drugs, attention must be paid to better prevention of hepatitis C transmission along with diagnosis, linkage to care, and access to treatment. This includes increased access to medication‐assisted treatment of opioid use disorder.
