{"doi":"10.1002/alz.14466","title":"Terminology controversies","abstract":"To the Editor, Recently published Alzheimer's Association (AA) revised criteria for diagnosis and staging Alzheimer's disease (AD)1 and neuronal synuclein disease (NSD) criteria2 evolved nearly simultaneously. The two independent groups of investigators in different neurodegenerative disease areas settled on remarkably similar conceptual approaches to diagnosis and staging. The AA workgroup was enthusiastic about the NSD approach because of the parallels between the two sets of criteria. However, as pointed out by Lang et al. we appreciate that the term NSD is not widely accepted at this time in the Parkinson's disease/dementia with Lewy body disease (PD/DLB) field, and we hope that this letter will correct any false impressions that our article may have created in this regard. Disease-modifying treatments have been approved for AD3, 4 and this puts the necessity for a biological definition and staging of AD on firm footing because therapy directed toward a biological target requires confirmation of that biology in the patient. Disease-modifying treatments have not yet been approved in the PD/DLB field although efforts are obviously continuing. The impetus for biologically based lexicon thus may not be as strong at this time in the PD/DLB field as it is for AD. It was not the intent of the AA workgroup to insert itself into terminology controversies in the PD/DLB field, and we acknowledge that here. Clifford R. Jack Jr. is employed by Mayo Clinic.
