{"doi":"10.1002/9781119876007.ch166","title":"Transplantation of Pancreatic Islets","abstract":"Transplantation of purified human pancreatic islets has become a viable treatment option for selected patients with type 1 diabetes. Phase III trials have documented the efficacy and safety of transplantation of allogeneic islets in restoring near-normoglycemia and protection from severe hypoglycemia in islet-alone and islet-after-kidney recipients. Health authorities in several countries have approved human allogeneic islet cells for the treatment of type 1 diabetes. The transformative experience of recipients of successful islet transplants has sparked increased commitment to research to overcome the remaining challenges. Islet banking has been optimized by vitrification and refined islet implantation and cellular cotransplantation approaches have been developed to promote islet cell engraftment. Substantial progress has been made in the development of an unlimited supply of islet cells for transplant, specifically with stem cell-derived islet cells now being evaluated in clinical trials. To circumvent the risks associated with chronic immunosuppression, several strategies have been advanced, including novel islet micro- and macroencapsulation technologies, induction of immune tolerance, and generation of hypoimmunogenic islets by gene editing.
